Award for Social Prescribing Link Worker of the Year - Social Prescribing Awards 2024

Matt Day, a dedicated social prescriber from Rural South Somerset, has been honoured with the prestigious 'Social Prescriber of the Year' Award in 2024. Since setting up his team in 2021, Matt has been a transformative force in the community, known for his grassroots approach and his ability to impact multiple levels of the social prescribing ecosystem. 

Jenny Hartnoll, a colleague who nominated Matt, said: “Matt’s passion for social prescribing goes beyond mere signposting. He educates higher-level stakeholders on the true essence of social prescribing, emphasising its role as a crucial component of a larger support system.  

“His efforts have not only kept VCFSE (Voluntary, Community, Faith, and Social Enterprise) groups thriving but have also fostered collaboration.” 

In the past year alone, Matt has launched several impactful initiatives. He set up the Crewkerne Timebank, a group of 25 members who volunteer to help each other with everyday tasks, promoting independence and reducing isolation. He also developed a Beginner Health Walk for patients with limited mobility and started a Men’s Wellbeing group that meets weekly for non-contact sports and mental health discussions. Additionally, Matt is working on a therapeutic growing project in collaboration with a Practice Nurse, connecting young families with nature and teaching them how to grow their own food. 

“Matt is a force to be reckoned with,” says Jenny. “He works tirelessly at all levels, building relationships and making a profound impact. He is modest, compassionate, and a true role model, especially for men entering the social prescribing workforce.”
