How social prescribing can supportpeople living with long-term conditions

Social prescribing is an increasingly established approach within primary care for supporting people struggling with things like loneliness, isolation or other complex social issues that affect their wellbeing. 

But social prescribing has also been shown to be very effective in supporting patients with the management of conditions such as diabetes, cardiovascular problems, dementia and cancer. With around 25 million people in England living with a long-term condition, and a round a quarter of us having two or more of these, social prescribing has a huge potential to benefit both individuals and our health system.
