    Forest of Hearts

    Forest of Hearts run weekly outdoor, friendly and purposeful sessions to create and maintain gardens in local hospitals, hospices, GP Practices and Community Centres. Our small and sociable groups offer opportunities for people to meet, make friends and learn new skills whilst being part of a local community that cares about the natural environment and its impact on health and wellbeing.

    Vision 4 Youth

    Vision4 Youth run a Youth Social Prescribing service for those aged 11-18. As a VCSE organisation, we are able to accept referrals from anyone - schools, social services, community groups and parents, as well as clinical referrals. We signpost to activities in the local area, as well as providing a befriending service for young people who need a confidence boost as their main focus.

  • Connect North - Northern Health and Social Care Trust

    Connect North facilitates a stepped approach social prescribing at the level required by the individual. Clients can access our directory of services for themselves or on behalf of a loved one (self-service) or be assisted/provided with information from the directory by another party (signposting). Where self-service and signposting have not been appropriate/effective clients can be referred to the Connect North Link Worker service for tailored 1:1 support (referral) using a validated assessment of wellbeing and co-created support planning. Client progress and outcomes are monitored with reporting for improvement. We have a growing network of community connectors who can signpost people to the directory alongside community information points where people can go to gain access to devices and skills to use the directory. Our Link Workers also host drop-in sessions within our local communities where residents can meet their local link worker without the need for a referral or appointment.

    Ways to Wellness

    SPACE CYP (Social Prescribing And Community Resources for Children and Young People) was developed collaboratively with Dr Anna Basu as a partnership project between Ways to Wellness, the Great North Children's Hospital and Newcastle University. More than half of all families with a child with complex healthcare needs, such as those resulting from neuro-developmental impairments and conditions, have financial difficulties; nearly half express unmet non-medical needs, and a third express difficulty accessing non-medical services (Kuo D.Z. et al, 2011). SPACE Pilot tests the impact of whole family psychosocial support for children with complex, chronic conditions - integrating the hospital with community settings.

    Come Singing

    Come Singing works with Norfolk Library Service to provide three singing sessions each month in Norwich for people living with dementia. Friendly, inclusive and slightly anarchic sessions take place front of library issue desks and are clearly visible and audible, often drawing in other library users. There is a enthusiastic attendance of between 35- 50 people. After gentle vocal and physical warm-ups, songs in a variety of genres, moods, speeds and keys offer cognitive challenge as well as fun and emotional expression. Library staff are not directly involved in sessions but set out the area beforehand and help readily with seating for special needs, enlarging texts, etc. Norfolk Library Service was vital in sustaining the dementia singing community during Covid lockdowns, hosting Come Singing videos on their website, and organising conference calls bringing small groups of singers together. Come Singing sessions are by far their best--attended events.

    Positive Directions

    Our project, one of 11 NHSE Health and Justice Vanguard initiatives, explores how to better support children and young people who have experienced trauma. Early consultation with young people highlighted a strong desire to feel connected to their communities and activities that promote belonging and wellbeing. In response, we made Social Prescribing a central focus—examining both its benefits for young people and how its application differs from the traditional adult, GP-based model. Our innovation lies in broadening the understanding of Social Prescribing: shifting it from a specialist service to a shared approach that any professional working with children and young people can embed in their practice. By equipping practitioners with trauma-informed tools to link young people to meaningful opportunities and community connections, we aim to make positive outcomes more sustainable and empower young people to rebuild trust, confidence, and connection.
