This session shares practical tips, real-life examples, and different perspectives on how social prescribing can better support people living with long-term conditions. It looks at why it is important to see the whole person, not just their health condition, and how working closely with other professionals can improve support.
The session will help you understand the challenges people with long-term conditions face and how link workers and others in similiar roles can respond in a personalised and holistic way.
By the end, you will have a better understanding of people’s lived experiences, learn useful ideas and examples you can use in your work, feel more confident providing person-centred support, work more effectively with other professionals, and better recognise the social factors that affect people’s health and wellbeing.
