Calling all link workers – we want to hear your story!

As part of our ongoing work promoting social prescribing, we are gathering case studies to help us illustrate the positive impact your work has on patients and the healthcare system at large. They will be shared with health leaders, commissioners, primary care staff and other link workers, among others, to demonstrate the crucial role that social prescribing can play.

The case studies need to demonstrate the four main themes outlined below:

  • IMPACT: Can you use your experience to explain how the link worker role functions and how it fits into a multidisciplinary team? You’d also need to demonstrate impact this role has on the health and wellbeing of patients (ideally referring to specific examples, with quotes)
  • DEVELOPMENT: Let us know how you have been supported to develop your skills and share examples of the ways in which this has positively impacted your work. Eg. Describing how your induction or a training course enabled you to support a patient’s particular needs.
  • SOCIAL SUPPORT: Share your experience of how linking patients to financial advice, housing support or other advisory services has improved a patient’s wellbeing.
  • PHYSICAL ACTIVITY: Tell us an example that shows the impact of physical activity on a patient’s health mental and physical health.

In the email, please use specific examples where possible to help clearly illustrate the breadth and scope of the work you do.

Taking part in a case study is likely to take around half a day of your time, and we can offer small care packs as a thank you for being involved.
