      Charlton Athletic Community Trust are the commissioned provider of a Social Prescribing service for the Royal Borough of Greenwich. We are currently advertising for Live Well Care Co-ordinators to work within our Social Prescribing Team to identify and work with people, including the frail/elderly and those with long-term conditions, to help co-ordinate care and support across health and care services.
